PARENT AGREEMENT

I hereby release and agree to hold JMG SportsWise harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the company, or that may otherwise arise in any way in connection with any services received from JMG SportsWise.

I understand that this release discharges JMG SportsWise from any liability or claim that I, my heirs, or any personal representatives may have against the company with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from JMG SportsWise. This liability waiver and release extends to the company together with all owners, partners, and employees.

ATHLETIC AGREEMENT

Participation in all sports can be inherently dangerous and, regardless of the care taken to avoid injury, it is impossible to ensure the safety of all participating individuals. In fact, many sports come with inherent dangers. Playing sports can demand cardiovascular fitness, coordination, and agility. Players may spend more than 45 minutes in intense physical activity and must be constantly aware of their surroundings (other players, goalposts, net posts, and the ball itself). Although soccer, basketball & volleyball are not generally thought of as a contact sport, at competitive levels, it can be. Although many risks can be avoided, some simply cannot.

While playing sports, accidents do happen. Some of these accidents may be minor like collisions with other players, collisions with the ground, and being struck by the ball. Other accidents may be more significant like collisions with the goalposts. Other injuries may also include:

Minor injuries like scrapes, bruises, strains, blisters, and sprains;
More serious injuries liken broken bones, concussions, muscle tears, and ligament tears;
Catastrophic injuries like heat stroke, cardiac arrest, and death

To reduce the risk of injury, participants are expected to abide by the following rules:

All participants are expected to be physically fit before participating in JMG SportsWise Programs
All players must adhere to the rules of the sports, thus creating a safe playing environment.

The above-mentioned participant agrees to follow the preceding safety rules, all posted safety rules, and all rules common to the enrolled sport. Further, the above-mentioned participant agrees to report any unsafe practices, conditions, or equipment to the JMG SportsWise Coach.

I certify that the above-mentioned participant:

Possesses a sufficient degree of physical fitness to safely participate.
Understands that he/she/they is to discontinue activity at any time he/she/they feels undue discomfort or stress
Will email JMG SportsWise (info@jmgsportswise.com) if there are any special needs or concerns you would like them to be aware of regarding the student.

Conduct:

JMG SportsWise has zero tolerance for lack of respect for officials, other cultures, religious institutions and ways of life by its players, coaches, parents or fans.
Students must abide by the Rules of Play and each of the individual location’s specific rules and regulations.

JMG Aftercare Walking Permission (Altadena Locations Only)

I understand that JMG Coach will pick up my child at OCS after school and accompany them to Loma Alta Park unless written permission is given affirming that they can walk on their own. I understand that a parent or guardian will either pick them up at the park after class is over or my child will be brought back to their aftercare program on school campus (If they are enrolled i.e. Boys and Girls Club) I understand that participation in walking to and from the park could involve risk of physical injury, illness, death or property loss, and despite safety precautions, OCS and JMG cannot guarantee safety thereof, as all risks cannot be prevented. JMG Inc, does not provide health and accident insurance for trip participants, and I understand that any medical expenses, property loss, or other personal expenditures that result during or from this travel/trip, are to be borne by the student/participant, or by their parent or guardian (if student/participant is a minor). I also hereby consent, give authorization to, and release from liability; trip leaders to secure any emergency medical treatment in event I am unable to, and I agree to be responsible for the costs thereof.

JMG Make Up, Refund and Cancellation Policy

Each Player purchases a season. Should JMG cancel the season entirely, a full refund or credit towards the following season will be offered. Should a class be postponed by JMG INC., extra time or dates will be added to the season and parents will be notified. Any other missed classes by players will not be credited or refundable. Make ups can be taken if there is another day the same sports classes are offered. Please make arrangements with us.

I have read, understood and agree to the Athletic Agreement and will fulfill my commitment to all, Athletic, Behavioral and Financial responsibilities. I fully support the policies of JMG SportsWise.
